Below are the Camper Forms for Summer 2010. Many of the forms need your attention now, so please be sure to print all of the forms and read through everything very carefully. To view and complete these forms, you will need the free Adobe Acrobat Reader which can be downloaded here.

Many of these forms can either be printed and returned after filling them out by hand or they can be submitted electronically.

Camper Parent Information
This packet is filled with information about travel, packing, contacting us, and other important details regarding the summer. Please read this entire packet carefully (especially the section on travel) – there have been changes from past summers.

These must be filled out and returned by April 20. Your camper may not attend camp unless all forms are on file in advance.

Administrative Form
For all campers. Friendship requests, travel information, bar/bat mitzvah tutoring request, permissions, and more.

Camper Health Form
For all campers. The health form should be filled out by you and your child's health care professional. Please keep a copy of this information for your files.

CampMeds
For all campers taking medication. If your child takes any medication, you must use the CampMeds service.

Parent Questionnaire
For all campers. Providing honest and accurate information about your child is crucial to ensuring his/her physical and emotional safety this summer.

Parent Agreement
For all campers. Please make sure to discuss expectations with your child prior to signing this agreement.

Covenant of Behavior
For all campers. This form should be signed by you and your child.

Internet & E-mail Agreement
For all campers.
